A shocking discovery in Diablo IV has left players both thrilled and confused. During the current Season of the Construct, a powerful developer test item named “[PH] Ichorous Salvation” has been found in loot drops. Originally meant for internal use only, this item slipped into the live servers unintentionally.

Despite the placeholder “[PH]” tag, the item is fully localized across all supported languages and comes with a complete in-game description. Its polished state suggests it was being prepared for future content—perhaps an upcoming patch or seasonal event. Yet, for reasons still unclear, it is now accessible to the player base.
Once equipped, this rogue-exclusive item grants players the ability to trigger “Barrage” on enemies affected by Damage over Time. More importantly, each trigger stacks additional “Barrage” hits without any upper limit, unlike standard item mechanics. This leads to astronomical levels of damage, often eliminating even the toughest foes instantly.

Blizzard has not yet responded to this unintentional leak. In the meantime, players lucky enough to loot the item are wreaking havoc across Sanctuary, turning elite bosses into dust with a single strike.
